When you smile, a thin band of muscle under the lower lash line bunches into a small roll that Koreans call aegyo sal. Aegyo sal filler lays a fine thread of hyaluronic acid gel along that band so the roll stays visible at rest. At Pixelab Beauty Clinic the gel is charged per 1 cc, and a same-day flight home is possible.

Is it for you?

Who aegyo sal filler suits, and who should skip it

Aegyo sal is the fullness of the pretarsal part of the orbicularis oculi, the ring muscle around the eye, where it lies right under the lashes. In East Asia it is a sought-after feature linked with a friendly, youthful expression, and a review of anatomy-based technique stresses that placing filler in the correct tissue layer there is what keeps the result natural and limits complications (Hong, 2025).

Two other under-eye features get mixed up with it. Eye bags are fat bulging lower down, toward the cheek, and they make a face look tired. The tear trough is the groove that runs below them. Aegyo sal sits higher and hugs the lash line. Adding a roll above a large bag stacks one bulge on another, which is why bags appear on the second list.

Some people want a roll that shows only when they smile, and others want it visible at rest as well. Both are design choices about the height and length of the roll, and they are drawn with you smiling at a mirror.

Usually a good fit

  • Little or no roll under the lashes when you smile
  • A roll that shows when smiling but flattens at rest
  • One roll visibly fuller than the other
  • Firm under-eye skin without large bags
  • Preference for a change that can be dissolved

We may suggest waiting

  • Large, bulging eye bags or heavy puffiness
  • Under-eye swelling that comes and goes with fluid
  • Pregnancy or breastfeeding
  • A rash, infection or cold sore near the eyes
  • A reaction to filler or hyaluronidase in the past
  • Permanent filler already placed under the eyes

How it works

How aegyo sal filler is designed and injected

Hyaluronic acid (HA) is a sugar molecule the skin produces on its own. In filler form it is cross-linked into a gel that the body breaks down slowly, and the enzyme hyaluronidase can dissolve it. That reversibility is a real advantage in a spot this visible: a roll that turns out too full can be reduced.

Proportion decides how natural the result looks. The roll follows the curve of the lash line and tapers toward the outer corner, and its height stays small next to the eye opening. A roll nearly as tall as the eye reads as swelling, and a long one that runs past the outer corner can make the eye look smaller. A tiny change in this spot alters the whole expression, the kind of small, precise adjustment the clinic's name points to.

The gel goes in through a fine needle or a blunt cannula, in very small amounts per pass. Placed too deep, it can drift downward and add fullness to the bag area. Placed too shallow, it can show as a ridge or a bluish tint. Blocked blood flow is the serious risk with any facial filler; in a large multicenter cohort it was very rare with both tools, and microcannulas were linked with fewer blockages than needles (Alam, 2021). A meta-analysis of under-eye HA trials found that the pattern of side effects differed by tool, with bruising tied to needles and swelling to cannulas (Nikolis, 2023).

The price list carries four filler product pairs, each charged per 1 cc, and it does not assign a product to this area. A softer gel suits a fine roll under thin skin, and starting with a modest amount leaves room to add at a review instead of removing excess.

1

Smile check

At consultation

Both rolls are compared at rest and while smiling, under even light.

2

Drawing the roll

Length and height are marked along each lash line, tapering at the outer end.

3

Numbing first

Cream is applied under both eyes before any injection.

4

Small-volume passes

Gel is laid in thin passes and molded gently, with symmetry checked between sides.

5

Review if needed

2–4 weeks later

Once swelling has gone down, the roll can be assessed and topped up if needed.

Prices

Aegyo Sal Filler Cost in South Korea

The cost of aegyo sal filler is the per-cc rate of the chosen product pair multiplied by the gel used for both rolls. Because the roll is a narrow strip, the cc figure on your quote moves the total more than the grade name does.

None of the listed figures include tax. Korea's 10% VAT is charged at payment, and each price card shows the VAT-inclusive amount next to the base rate, so the figure you see is the figure you pay.

Three of the four pairs carry the words "retouch available" on the price list. What a retouch covers and by when are not printed, so get those terms in writing. Reducing a roll with hyaluronidase has two lines: one for filler placed at this clinic and one for filler injected at another clinic, where the product may be unknown.

Safety

What can go wrong with aegyo sal filler

Skin under the lashes is thin and moves with every blink, and that shapes the list:

  • Swelling and bruising for 1 to 2 weeks, in the clinic's guide.
  • An overfilled roll that looks puffy or sausage-shaped, most visible when smiling.
  • Beading, small lumps or a visible ridge along the lash line.
  • Rolls of different size or length on the two sides.
  • A bluish tint where gel sits too close to the surface (Tyndall effect).
  • Gel drifting downward and adding fullness to the bag area.
  • Blocked blood flow in a vessel and, very rarely, vision loss.

In a review of published vision-loss cases after cosmetic filler, the glabella and nose were the injection sites behind most reports, and the authors called for strategies to cut the risk wherever filler is used (Kato, 2022).

Get in touch with the clinic straight away if the skin below the eye turns pale, gray or blotchy, if pain keeps building, or if your vision blurs, doubles or darkens. Hyaluronidase breaks down HA gel, and acting quickly matters when a vessel is involved.

FAQ

Aegyo sal filler questions

What is aegyo sal, and is it the same as eye bags?

No. Aegyo sal is a slim roll of muscle right under the lashes that grows fuller when you smile. Eye bags are fat that bulges lower down, toward the cheek. Filler can build up the roll, while gel added over a large bag tends to make the area look heavier.

Will aegyo sal filler make me look like I am always smiling?

Not when the roll is kept in proportion. A slim roll reads as soft fullness at rest, while a tall or long one can look swollen. Starting with a smaller amount and adding at a review 2 to 4 weeks later keeps the shape under control.

What is the downtime after aegyo sal filler?

Swelling and possible bruising last 1 to 2 weeks in the clinic's guide. The number of entry points, the thickness of your skin and how easily you bruise decide where you fall in that range. Skip eye rubbing and heavy eye makeup at first, and report any pale or dusky patch at once.

Can I fly home on the day of aegyo sal filler?

Yes. The clinic's guide allows a flight home on the day of injection. A review, when needed, comes 2 to 4 weeks later, so anyone who wants the option of a top-up can time a return trip around it. Sleeping face-up on the plane keeps pressure off the fresh roll.

How long does aegyo sal filler last?

About 6 to 18 months is the range the clinic gives for filler, varying with the product and the area. The roll sits on a muscle that moves with every blink and smile, and the rate at which gel breaks down differs between people. It fades gradually.

Who is the doctor at Pixelab Beauty Clinic?

Dr. Jongkeun Song, the Medical Director, is a Korean board-certified plastic surgeon. Residency in plastic surgery at The Catholic University of Korea, Seoul St. Mary's Hospital included the muscles and vessels around the eye socket, the region aegyo sal filler works in.

Are aegyo sal filler prices listed before VAT?

Yes. Every figure excludes Korea's 10% VAT, which is added when you pay. One cc of Belotero or Lorient, for example, is ₩360,000 before VAT and ₩396,000 with it. Your quote multiplies the rate by the gel used on both sides.

Can aegyo sal filler be dissolved if the roll looks too full?

Yes, if the gel is hyaluronic acid. Hyaluronidase breaks it down when a roll looks puffy, lumpy or uneven. It is listed at ₩150,000 + VAT for filler done here and ₩200,000 + VAT for filler injected at another clinic, so bring the product name if you know it.

Does aegyo sal filler hurt?

Numbing cream goes on before the injections. What remains is pressure and a brief sting at each entry point under the lashes. The area can feel tender while swelling settles. Mention any earlier reaction to numbing creams before the cream is applied.

Can I consult in English?

Yes. The clinic has English interpreters on staff, and before booking you can send a relaxed and a smiling photo of your eyes to our WhatsApp with your questions in English. We will get back to you as soon as possible.

Do I need a deposit for aegyo sal filler, and what if I cancel?

Deposit and cancellation terms are not listed on this page. Ask for them in writing together with your quote, and check how a change of flight dates is handled before you transfer any money.
